New Patient Forms

We require all new patients complete our Dental and Medical History forms prior to your new patient exam and dental hygiene appointment.

Please arrive 15 minutes early to your new patient appointment. We will have you fill out and complete these new patient forms at this time. We also have our Dental and Medical History forms available for you to print off and bring in for that initial appointment. If you do print these forms out, please complete them in full, sign and date both forms, and bring them in to our office prior to your appointment. Please click on the PDF file below and print.
